AHA Application for Admission - Mutual Help, Low Rent, and Senior Housing.

AHA Release of Information - Must be submitted with the Application for Admission, Family Composition and Income Update and Scholarship Application.

F4506-T Form - Allows AHA to request copies of your tax returns from the IRS. If you do not have copies of your tax returns, please include this form with any application you submit. Please keep in mind that it may take up to 45 days for us to get a response.Your application will not be considered complete without this information.

AHA Involuntary Displaced - Must be completed by a certifying agency i.e. Tribal / City Office or Public Safety and submitted with the Application for Admission.

Family Composition and Income Update - Used to update information for current program participants and/or wait-listed applicants.
