Home Modification Rehabilitation Program

Purpose

The Home Modification Rehabilitation Program provides housing modifications for Alaska Mental Health Trust beneficiaries and individuals with disabilities or special needs residing within the Aleutian/Pribilof Islands Region. This program is provided in partnership with Alaska Division of Health and Social Services and Alaska Mental Health Trust Authority.

Description

Grant:  Home Modification Rehabilitation may be used for accessibility improvements and other modifications that mitigate the functional limitations imposed by the conditions of the beneficiaries or individual's special needs. All improvements must relate to the beneficiary's or individual's special needs / qualifying disability.

Maximum Amount:

1. $16,000 for single-family homes.
2. $10,000 for rental property.

Program Eligibility

• Home location within Aleutians East and West Area of Alaska.
• Beneficiary or individual with special needs must occupy residence during retention periods.
• Where the modification is provided in a personal home, the owner must be willing to sign a Promissory Note for retention for a period of three years.
• Modifications of rental property with costs up to $2,500 require a 1-year lease. For costs over $2,500 a 2-year lease is required.
• This program will be administered under the guidelines of Aleutian Housing Authority's Home Loan Repair program within the following exceptions:
  • No income restrictions
  • No ethnicity restrictions
  • No interest accrued
  • Applicable program limits
  • Applicable retention periods
• Description of accessibility or mobility modification needed.
• Name of resident in home that will benefit from the modification.

Property Requirements

• Lead based paint prevention requirements apply.
• Aleutian Housing Authority or a licensed and bonded contractor must perform all work funded under this program.
• Before, during, and after pictures showing scope of work must be included in file for each project.
• If beneficiary no longer occupies the residence within the 3 - year retention period, all grant funds used for the modification rehabilitation purpose of the grant must be repaid to the State of Alaska.

Eligible Repairs

Eligible Modification Rehabilitation funded by this program includes but are not limited to:

• Stairway or ramp installation or modification.
• Widening of doorways and hallways.
• Adjustments and adaptations to improve mobility within the interior living space.
• Adapting the environment to manage behavioral issues associated with Alzheimer's disease and related disorders.
• Amplification, visual devices, and signaling devices to assist hearing and / or visually impaired individuals (i.e. equipment for stoves, windows, thermostats, etc.).
• Modifications that assist in performing daily tasks.
• Lease and / or equipment for trial use from the Assistive Technology Library of Alaska (ALTA).
• Other housing modification specifically related to the beneficiary's or individual's qualifying disability and approved by the Department of Health and Social Services.
